Over the years, we have worked with many commissioners and gained valuable insight into the various challenges associated with evaluating and implementing new care packages. These challenges may arise from hospital discharges, breakdowns in current care provision, or urgent care needs.
We work to ensure Service Users get to live where they want, how they want. We will work with the Service User, the family, the commissioners and their wider MDT to build a safe, effective, caring and well-led package of care around any Service User with any complexity of care need.
We are led by your timeframes – whether these are dictated for medical reasons, the pressures of bed blocking or for service user-led timeframes such as getting home for Christmas or the kid’s summer fete. We will work with the Service User, the family, the commissioners and their wider MDT to ensure that your timeframes are met safely and realistically.
The NCC is comprised of a team of skilled individuals, who have refined our referral and implementation process, ensuring a smooth transition for each Service User. By centralising this process, we ensure that our delivery is consistent.

To ensure we meet the stipulated timescales Sugarman Health & Wellbeing will provide 24/7 accessibility, via the local office and a 24/7 on-call service, who have access to all patient information and will refer all enquiries/referrals to the NCC Team.
