Our blueprint to success

We believe there are 4 key principles to successfully delivering care. We have refined and perfected this process and call it the Sugarman Way.

Our blueprint to success follows 4 steps that are followed throughout the implementation process and allow us to personalise the mobilisation of care packages to ensure our Service Users receives a safe and consistent service from the start.

Our 4 key principles

Refer

A referral is received, and this document is then reviewed by our operational and clinical team to ensure that we are confident we can implement safely and efficiently, and a response is given back within the requested time from the commissioner. We work closely with our commissioners to understand their needs, expectations and timeframes.

Implement

We create a bespoke implementation plan which can be shared with all key stakeholders. This working document will demonstrate tasks from recruitment, assessment and care planning, training and all associated operational tasks. We are happy to provide regular updates to all stakeholders either in writing or at regular meetings

Onboard

We will recruit to person and skill specifications, personalise the introductions and training to each individual and ensure all carers adhere to our onboarding process. The package of care will be onboarded to the timeframes set with the MDT and individual.

Retain

Regular review and update calls are made in weeks 1,4 and 8 with the Service User, family, commissioner and MDT’s involved. We set and review desired outcomes with the Service User to ensure we are reaching them. Ongoing collaboration work to build relationships with the branch, Service User and carers and review of competencies to ensure carers are working within their scope of practice.

Our packages

Our packages are tailored to the Service Users specific needs, wishes and lifestyle choices. In developing care plans, we explore with the Service User, their family and commissioners outcome focused goals and by ensure these are worked toward in all aspects of care planning and delivery.
